Who Qualifies for a Moment?
- Any patient in the Quincy Medical Group’s geographic service area (75-mile radius around Quincy, IL).
- Patients diagnosed with a terminal illness.
- Patients receiving memory care with a diagnosis such as dementia or Alzheimer’s disease.
- Patients with a health diagnosis that impacts their day-to-day life.
- Patients who are under supervision of a health care provider or an examination may be required.
- Patients who have not received a wish fulfillment from any other organization.
What Qualifies as a Moment?
We only ask that the request be truly what the nominee desires. Our team spends time with the nominee and their family to learn about them and their hope for their experience. We understand that those we honor are facing terminal illnesses and time is precious. With that understanding, we can work with the nominee and family to coordinate moments within a short timeframe and while the patient is undergoing care with their health team. The Moments That Matter team, thanks to the support of our generous donors, has everything covered — fully coordinating and funding an entire experience.

About Moments That Matter
Our everyday moments seem simple to us. It’s those moments that are so easy to take for granted, but hold the most meaning. We invite you to join us as we create memories and honor the lives of our nominated individuals.
Our Story
Moments That Matter was launched in 2018 in the Quincy area as part of the UnityPoint Hospice Program, focusing on honoring patients by fulfilling their “bucket list wishes.”
As we met with patients and their families, we found they were so overwhelmed with worry and challenges, that the desire for other experiences was not always a top priority. Moments That Matter changed that by talking to patients and families about experiences they would like to share before illness progressed to a point when that may no longer be possible.
In 2019, Moments That Matter expanded by joining with the Quincy Medical Group Foundation, giving us an opportunity to honor more people in our community with moments. In 2021, we took this opportunity to reflect on the past and look to the future of our fund, as many things were put on pause due to the pandemic. Now more than ever, we feel the importance of everyday moments.
As Moments That Matter has evolved, we continue to honor individuals facing a terminal illness. The fund has expanded to include patients in memory care due to Alzheimer’s disease or dementia, patients facing difficult diagnoses, and legacy moments for those who are nominated through Moments That Matter but who pass away before seeing their moment realized.
Along the way, as moments are granted, we hope to involve more of our local communities so those we honor can feel the love of those around them.
Nominate Someone for a Moment
Generosity is entering into someone else’s story. It’s connecting our life story with theirs. Generously giving to someone in need or dedicating time to listen to their concerns allows us to step into their world, fostering a connection between us. With each wish fulfilled, we are honored to be a part of the story of our patient’s life and everyone that gives so much to make them happen.
While donations fund each special moment created, oftentimes we need things other than money. Maybe you can give of your time, or you can lend us something to help make a wish come true. All moments are coordinated with Pediatrics, Behavioral Health, Nursing, and Oncology departments. Someone can also be nominated by a parent or loved one with a health information release.
