Who Qual­i­fies for a Moment?

  • Any patient in the Quin­cy Med­ical Group’s geo­graph­ic ser­vice area (75-mile radius around Quin­cy, IL).
  • Patients diag­nosed with a ter­mi­nal illness.
  • Patients receiv­ing mem­o­ry care with a diag­no­sis such as demen­tia or Alzheimer’s disease.
  • Patients with a health diag­no­sis that impacts their day-to-day life.
  • Patients who are under super­vi­sion of a health care provider or an exam­i­na­tion may be required.
  • Patients who have not received a wish ful­fill­ment from any oth­er organization.

What Qual­i­fies as a Moment?

We only ask that the request be tru­ly what the nom­i­nee desires. Our team spends time with the nom­i­nee and their fam­i­ly to learn about them and their hope for their expe­ri­ence. We under­stand that those we hon­or are fac­ing ter­mi­nal ill­ness­es and time is pre­cious. With that under­stand­ing, we can work with the nom­i­nee and fam­i­ly to coor­di­nate moments with­in a short time­frame and while the patient is under­go­ing care with their health team. The Moments That Mat­ter team, thanks to the sup­port of our gen­er­ous donors, has every­thing cov­ered — ful­ly coor­di­nat­ing and fund­ing an entire experience.

About Moments That Matter

Our every­day moments seem sim­ple to us. It’s those moments that are so easy to take for grant­ed, but hold the most mean­ing. We invite you to join us as we cre­ate mem­o­ries and hon­or the lives of our nom­i­nat­ed individuals.

Our Story

Moments That Matter was launched in 2018 in the Quincy area as part of the UnityPoint Hospice Program, focusing on honoring patients by fulfilling their “bucket list wishes.”

As we met with patients and their families, we found they were so overwhelmed with worry and challenges, that the desire for other experiences was not always a top priority. Moments That Matter changed that by talking to patients and families about experiences they would like to share before illness progressed to a point when that may no longer be possible.

In 2019, Moments That Matter expanded by joining with the Quincy Medical Group Foundation, giving us an opportunity to honor more people in our community with moments. In 2021, we took this opportunity to reflect on the past and look to the future of our fund, as many things were put on pause due to the pandemic. Now more than ever, we feel the importance of everyday moments.

As Moments That Matter has evolved, we continue to honor individuals facing a terminal illness. The fund has expanded to include patients in memory care due to Alzheimer’s disease or dementia, patients facing difficult diagnoses, and legacy moments for those who are nominated through Moments That Matter but who pass away before seeing their moment realized.

Along the way, as moments are granted, we hope to involve more of our local communities so those we honor can feel the love of those around them.

Nominate Someone for a Moment

Gen­eros­i­ty is enter­ing into some­one else’s sto­ry. It’s con­nect­ing our life sto­ry with theirs. Gen­er­ous­ly giv­ing to some­one in need or ded­i­cat­ing time to lis­ten to their con­cerns allows us to step into their world, fos­ter­ing a con­nec­tion between us. With each wish ful­filled, we are hon­ored to be a part of the sto­ry of our patient’s life and every­one that gives so much to make them hap­pen.

While dona­tions fund each spe­cial moment cre­at­ed, often­times we need things oth­er than mon­ey. Maybe you can give of your time, or you can lend us some­thing to help make a wish come true. All moments are coor­di­nat­ed with Pedi­atrics, Behav­ioral Health, Nurs­ing, and Oncol­o­gy depart­ments. Some­one can also be nom­i­nat­ed by a par­ent or loved one with a health infor­ma­tion release.
