Patient Assistance
One way the QMG Foundation wraps its arms around community members is through our Patient Assistance Programs. Facing a serious health event can be overwhelming. Day-to-day life doesn't stop, and there is added financial strain, forcing tough decisions. The QMG Foundation established its Patient Assistance Programs to alleviate financial burdens by providing support with housing, auto, utility, and phone bills.
What is the Patient Assistance Program?
The QMG Foundation Patient Assistance Programs provide financial assistance with household bills to patients experiencing a health-related financial hardship, as assessed by a social worker, caseworker, or healthcare provider. The programs provide short-term assistance (no more than 3 months) with an agreement for a long-term strategy partnered with the patient care team.
What qualifies as a health-related financial hardship?
Recent health events: oncology treatment, injury/surgery/procedure that interrupts ability to work, new diagnosis requiring frequent care, or recovery taking longer than expected.
What type of assistance does the QMG Foundation assist with?
- We can assist with household expenses such as:
- Mortgage/Rent
- Utility bills
- Gas to specialty healthcare appointments (through gas cards)
- Groceries (through grocery gift cards)
- Car payments
- Phone bills
What does not qualify?
- We are unable to assist with expenses such as:
- Medical or insurance bills
- Cable bills
- Long-term or forward payment past 2-3 months
- Services that have been disconnected due to non-payment
- Defaulted payment agreements, outstanding loans or bad debt (including bills that have been sent to collections, or involved in any court processes or fees, etc.)
What is the process?
The QMG Foundation application process starts with a patient's healthcare team. When a patient experiences a health-related financial hardship, their healthcare provider or care team can refer them to a QMG Behavioral Case Worker, who helps them find community resources and assists those with a qualifying health event in completing a QMG Foundation application.
Oncology Guidelines
To be eligible, a patient must be currently undergoing treatment for oncology services. To receive assistance, the patient must complete an application and be in contact with a QMG Oncology social worker and/or financial counselor.
General Patient Assistance Guidelines
To receive assistance, the patient must complete an application, be in contact with a QMG Behavioral Health case worker, and actively reach out to community resources.
To ensure we have enough resources to assist all patients, the QMG Foundation reserves the right to allocate financial assistance to each patient based on available funding. Regardless of the support given, additional community resources will be provided to every applicant.
